Parental Consent for Schools to Administer MedicinePersonal Data Consent Form
Since the government changed the rules concerning holidays in term time, all such absences have to go down as unauthorised (this is out of school’s control – see our Attendance Policy for more information). However, in exceptional circumstances, absences from school may be authorised. The following form needs to be submitted two weeks prior to any known occurrences.
